harm updates

We made harm rolled each time its taken, and harm clock is now a clock for 'taken out' status.  harm move also only allows for Character Tags to aid as any Armor or such damage reducing or cover protecting [tags] should reduce Harm taken before the roll. 

also added note on how much [tags] add to a roll each, +1, oops can believe we forgot that!
